How do I reset my password?
Go to the Forgot Password page: https://hazmatalliance.org/forgot-password/.
Enter your registered email address or Usdot number and click on Get New Password button. You’ll receive an email with a link to reset your password. Follow the steps in the email to set a new password.
I didn’t receive the password reset email — what should I do?
First, check your spam/junk folder. If you still don’t see the email, make sure you entered the correct address on the Forgot Password page: https://hazmatalliance.org/forgot-password/. If the issue persists, contact our support team for help.
Can I reset my password if I no longer have access to my email?
For security reasons, password reset instructions are sent only to your registered email address. If you no longer have access to that email, please contact our support team and we’ll help you verify your identity and update your account.
Carrier Login Locked Out – How to Reset Access
If a carrier or service provider is unable to log in and receives a “locked out” or access error, follow the steps below to regain access.
Step 1: Confirm the Correct Login Page
Make sure you are logging in at the correct URL:
https://hazmatalliance.org/login/
Step 2: Use the Correct Credentials
- Username: Your USDOT number
- Password: Copy and paste exactly as provided (case-sensitive)
Step 3: Try an Incognito/Private Browser Window
Open a private or incognito browser window and attempt to log in again.
Step 4: Check for IP Blocking
- Go to whatismyipaddress.com
- Copy your IP address
- Provide it to support for whitelisting
Step 5: Account Reset / Unlock
Contact support and provide your USDOT number(s) and associated email address.

I can’t see the Print option—what should I do?
If the Print option is not visible, please make sure all required steps in the application process are marked as complete with a check mark. The Print option will only appear once every required step has been completed.

How can I manage multiple carrier accounts under one login?
You can request an Advisor/Corporate account, which allows a single organization to manage multiple carrier accounts. Please contact our support team to set this up.
Why is an extra review fee ($500) showing in my application?
The $500 review fee is part of our 3-year renewal process. Under this policy, the renewal fee is required for the first year, while the fees for the next two years are waived—as long as the application is submitted within the required time period.
If the application is not submitted on time, the renewal fee will be charged again.
